How to View Responses to a Questionnaire
There are various ways to view caller responses to a questionnaire

โก๏ธ Viewing Caller Responses
There are 4 ways to view caller responses to a Questionnaire that you created:
- SMS Notifications
- Email Notifications
- Responses Dashboard
- PDF Exports (print-friendly)
๐ SMS Notifications
- Log into your Dashboard
- Click your name (or avatar) in the bottom left sidebar to expand your User Settings
- Click on Notifications
- Toggle on SMS Notifications
- Input an number capable of receiving SMS texts and press the โ button to save
- Press the +Add SMS Number button to add additional SMS number(s)
- Then, scroll down below Notification Events and enable Questionnaire Completions
When enabled, you\'ll receive SMS notifications when a caller completes a verbal questionnaire. The SMS message will display the Question and Answer pairs.
๐ง Email Notifications
- Log into your Dashboard
- Click your name (or avatar) in the bottom left sidebar to expand your User Settings
- Click on Notifications
- Toggle on Email Notifications
- Input an email address and press the โ button to save
- Press the +Add Email button to add additional email address(es)
- Then, scroll down below Notification Events and enable Questionnaire Completions
When enabled, you\'ll receive email notifications when a caller completes a verbal questionnaire. The email message will display the Question and Answer pairs.
๐ป Responses Dashboard
- Log into your Dashboard
- Click on AI in the left sidebar to expand
- Click on Questionnaire
- In the Questionnaire page, select the Responses tab
- View caller responses to a specific questionnaire by selecting the questionnaire name in the Questionnaire dropdown -- or view all questionnaires
- Filter by date using the Date Range dropdown
- Scroll down to view the list of callers who completed the questionnaire
- Click the View button next to a specific caller to view the actual responses collected from that caller
๐งพ PDF Export (print-friendly)
- Follow the instructions provided above for viewing the ๐ป Responses Dashboard
- To export a print-friendly PDF list of callers who completed a questionnaire, click the Export button
- To export a print-friendly PDF list of responses from a specific caller, click the View button next to the caller and then click the Export PDF button
โ๏ธ How to Configure Questionnaire Triggers
- Click Expand Questionnaire Trigger Settings.
- To have Sloane always start your questionnaire when she answers a call, select Always Trigger at Start of Call.
- To trigger the questionnaire only when callers mention specific words or phrases, select Custom Keywords and enter your keywords in the Caller Keywords box:
- Separate each keyword with a comma.
- Include related keywords (e.g., โpriceโ, โpricingโ, โcostโ).
- Suggested related keywords may auto-populate as you type.
- Use the Keyword Sensitivity dropdown to set how strictly Sloane should interpret your keyword rules.
โฐ How to Create Questions
- Click Expand Questions.
- Under Q1, enter your first question in the Question input box.
- Optional: Add a Question Label to improve readability in your stored responses.
- Optional: Select whether the question is unconditional or conditional in the Question Type dropdown.
- Optional: Choose the required response type in the Expected Answer Format dropdown.
- Optional: Add a follow-up prompt in the Follow-up Prompt box.
๐ Why Add a Label to Each Question?
Adding a Question Label improves clarity and makes responses easier to read.
- Example responses without labels:
May I have your name, please? Alex SacksCan I have your phone number, please? 310-555-3829Are you interested in our services? Yes
Name: Alex SacksCallback Number: 310-555-3829Potential Customer: Yes
๐ฒ Purpose of the Expected Answer Format
- The Expected Answer Format trains Sloane on how long she should wait for a callerโs response.
- Each format type tells Sloane the response time needed before moving to the next question.
- Example:
- Question: "Can you spell your first and last name?"
- Caller spells their answer โก๏ธ spelling words require more time
- Therefore, choose Spelling of word(s) as the Answer Format
- Example:
- Question: "Are you 65 years old or older?"
- Caller responds โyesโ or โnoโ โก๏ธ this requires minimal time
- Therefore, choose Yes/No as the Answer Format
๐ฑ How to Configure Conditional Questions
- Conditional questions allow Sloane to ask different follow-up questions based on how a caller responds.
- Each conditional question begins with a primary question, then branches into different follow-ups depending on the callerโs answer.
- Use conditional questions to route callers down different paths (e.g., by age, urgency, or service type).
- Example:
- Question 1: "Are you 65 years old or older?"
- If the caller answers "Yes", Sloane asks Question 1A.
- If the caller answers "No", Sloane asks Question 1B.
By chaining multiple conditional questions, you can create deeper logic trees that guide each caller through a customized path.
